LESC BHCC Welcomes New Clinical Integration Coordinator Yelena Kaplan, MPA
She will serve as principle liaison to LESC BHCC member agencies in the implementation of referrals and service coordination agreements and clinical integration protocols to enhance service delivery to BHCC clients.
The BHCC partners Kaplan will help guide include mental health, SUD and physical health care facilities and community-based service providers. She will also be involved in using the new Care Coordination and Data Analytics Platform to facilitate partner integration in her new role.
